IVIG Shows Promising Results in Immunological Infertility and Recurrent Implantation Failure – Dr Nisarg Patel and raises hopes in infertile patients

Intravenous immunoglobulin (IVIG) therapy has emerged as a promising treatment option for couples struggling with infertility and recurrent implantation failure. To understand IVIG, we must understand the reasons for immunological infertility and recurrent implantation failure. Immunological fertility occurs when a woman’s immune system considers the embryo a foreign invader and attempts to destroy it, leading to miscarriage, implantation failure and other complications. 

Recurrent implantation failure refers to women with three failed IVF attempts with good-quality embryos. Within the range of multiple risk factors, if there is evidence of certain levels of cytokine and specific autoantibodies, it hints that the immunological factor is responsible for the failure of the IVF cycle. Therefore, consulting with an IVF specialist who is knowledgeable in immunology may be beneficial in addressing this issue.

For instance, anti-nuclear antibodies, anti-cardiolipin antibodies (ACAs), and anti-phospholipid antibodies are some autoimmune antibodies that can cause pregnancy loss. Also, abnormal levels of cytokines can cause miscarriages. Cytokines are proteins produced by the white blood cells to manage inflammation in the body. Research states that elevated levels of TNF-α, a type of cytokine, increase the chances of miscarriage and harm pregnancy. Similarly, several autoimmune antibodies, such as anti-nuclear antibodies, anti-cardiolipin antibodies (ACAs), and anti-phospholipid antibodies, are involved in biochemical pregnancy loss.

The human immune system comprises natural killer cells, whose role in a pregnant woman is to provide a conducive environment for placentation and uneventful pregnancy, but a high count of natural killer cells can cause failed embryo implantation and miscarriage.

 Now let us learn the role of Intravenous immunoglobulin (IVIG) in such patients. As per research, intravenous immunoglobulin G (IVIG) can suppress high activity levels of autoimmune antibodies, reduce the destroying ability of NK cells and also inhibit cytokines.
